Welcome to Ladé Med Spa, LLC (“we,” “our,” or “us”). By accessing or using our services, website, booking system, or visiting our facility, you agree to the following Terms & Conditions. Please read them carefully.

 General Information

Ladé Med Spa, LLC provides aesthetic, wellness, and skincare services. All treatments are performed by licensed or certified professionals according to state regulations. These Terms & Conditions apply to all clients, customers, and visitors (“you”).

 Medical Disclaimer

• Our treatments are non-medical, unless otherwise stated, and do not replace medical advice or care from a licensed healthcare provider.

• Some services may require a medical consultation or clearance prior to treatment.

• You agree to disclose accurate health information and notify us of any changes before every visit.

 Appointments & Scheduling

• Appointments may be booked online, by phone, or in person.

• A valid credit/debit card may be required to hold an appointment.

• You are responsible for arriving on time. Late arrivals may result in shortened service time or rescheduling.

Cancellation & No-Show Policy

To respect our providers’ time, we enforce the following policies:

• 24-hour notice is required to cancel or reschedule.

• Cancellations made less than 24 hours before the appointment may incur a fee.

• No-shows may result in a charge of up to 100% of the scheduled service cost.

• Repeated no-shows may require prepayment for future bookings.

 Payments

We accept cash, credit/debit cards, and approved payment platforms.

All service payments are due at the time of treatment.

Packages, gift cards, memberships, and prepaid services are non-transferable.

Refund Policy

• Due to the nature of cosmetic and wellness services, all services rendered are non-refundable.

• Prepaid packages or products may be refunded only if unopened and returned within 14 days, at management’s discretion.

• Results vary by individual; we make no guarantees regarding specific outcomes.

Results & Risks

• You understand that individual results vary based on skin type, lifestyle, health status, and compliance with aftercare.

• You acknowledge that some treatments may involve temporary discomfort, redness, swelling, or other mild side effects.

• You agree to follow all pre-treatment and post-treatment instructions provided by our staff.

 Minors

• Clients under 18 must have a parent or legal guardian present.

• Certain services may not be available to minors, per state law.

 Gift Cards & Packages

• Gift cards are non-refundable and may only be used for services or products at Lade’ Med Spa, LLC.

• Packages or series expire 12 months from the purchase date unless stated otherwise.

 Privacy & Consent

• By booking a service, you consent to the collection of personal, contact, and health information necessary to provide safe and effective treatment.

• We do not sell or share your information with third parties except as required by law.

Photography & Marketing

• With your consent, before-and-after photos may be taken for documentation or marketing purposes. Photos will never be used without your written approval.

 Right to Refuse Service

We reserve the right to refuse service to anyone for reasons including but not limited to:

• Medical contraindications

• Inappropriate behavior

• Failure to follow spa policies

• Safety concerns

 Liability Waiver

By receiving services, you acknowledge and accept the risks associated with aesthetic and wellness treatments.

You agree that Ladé Med Spa, LLC and its staff are not liable for:

• Allergic reactions

• Adverse skin responses

• Failure to achieve intended results

• Misrepresentation of your health information.

 Changes to Terms

Ladé Med Spa, LLC may update these Terms & Conditions at any time. Updates will be posted on our website with a revised “Last Updated” date.

 Governing Law

These Terms & Conditions are governed by the laws of the State of Illinois.
